| The Minority Applicant Pool System (MAPS) is authorized by the State of Tennessee to improve access to long-term care services for elderly individuals in Davidson and surrounding counties. MAPS addresses certain problems associated with minority groups obtaining long-term care services. The program seeks to remedy these problems through community educational programs designed to inform elderly citizens and others of long-term care options and admission requirements. Family counseling sessions and placement assistance prepare our clients and their families for long-term care placement. |
| Elderly individuals who reside in Davidson and surrounding counties and who are in need of long-term care are eligible for MAPS. Priority is given to residents who live in "communities at risk" or in neighborhoods where a large number of elderly individuals reside and are at risk of institutionalization. |
| The MAPS program is administered by MCHRA through State funding. The State regularly monitors and evaluates the program. MCHRA also ensures that MAPS is achieving its goal of providing assistance to the elderly of Davidson and surrounding counties. |
| What was done in 2003-2004? |
| MAPS continued in its efforts to identify and educate individuals who are eligible for the program. We met with families, individuals, groups and churches to share the purpose and goals of the program. We also assisted eligible residents who were on waiting lists for long-term care facilities in the Nashville area. We disseminate pamphlets containing information about Medicare, TennCare, Social Security, elderly abuse, home health care, resident rights and other issues relevant to helping the elderly live more fulfilling and healthier lives. Referrals were made to community service organizations, hospitals and other social agencies. |
